Bringing Care to Where Elders Already Live
In the heart of communities like Pattinapakkam, Hosali, and Jogeshwari, where many older persons live in neglect, isolation, and vulnerability, our Community Based Elder Support Program reaches elders with compassion, dignity, and consistent care. Through a dedicated door to door outreach model, our Elderly Supporters and field team identify senior citizens who are in need of emotional, social, medical, and functional support.
The Need
- Many elders in low income urban communities live without anyone regularly checking on their health, nutrition, or safety, without family support in some instances.
- Mobility issues, poverty, or isolation often prevent them from reaching a clinic or centre on their own.
- Without regular contact, health issues, abuse, or neglect can go unnoticed for months.
How It Works
Household Line Listing
We map every household in a target community, identifying where elderly residents live.
Door to Door Outreach
Our field team visits each identified household directly, reaching elders who would otherwise never approach a centre.
Individual Assessment
Every elder is assessed using a structured tool that looks at health, mobility, social support, and living conditions.
Care Categorisation
Each elder is carefully assessed and categorised based on the intensity of their needs: High Intensity, Moderate, and Low Support. Special focus is given to elders in the High Intensity category, who often face multiple challenges such as poor health, loneliness, mobility limitations, financial insecurity, abandonment, or lack of family care.
Individual Care Plan
For every identified elder, an Individual Care Plan is developed to address their specific needs and improve their overall quality of life. The team follows up closely, working to resolve the key issues affecting their wellbeing, whether healthcare access, emotional support, social entitlements, nutrition, mobility assistance, counselling, or community linkage.
Ongoing Support
- Elderly Supporters: trained community workers who visit regularly for companionship, monitoring, and basic assistance
- Physiotherapytherapy: addressed by our physiotherapists for elders with mobility or pain related needs
- Active Ageing Day Care: Beyond home based support, the programme promotes active ageing through a Day Care Model, where elders take part in recreational, cognitive, social, and wellness activities in a safe, inclusive space in Chennai, Bangalore, and Mumbai, offering companionship, engagement, dignity, and emotional support for elders who would otherwise remain socially isolated.
- Bedridden Elder Support: elders who cannot leave home continue to receive care and monitoring through regular home visits
Where We Work
- Chennai, including communities such as Pattinapakkam: 3,242 households line listed, reaching approximately 1,247 elders through door to door visits.
- Bangalore: 423 households line listed, reaching 279 elders through door to door visits
- Mumbai: Geriatric Day Care Centre Jogeshwari, offering the same structured day care, meals, and companionship to around 132 elders.
Who Can Benefit
- Vulnerable elders in low income urban communities in the selected slums of Chennai, Bangalore, and Mumbai
- Elders with mobility limitations or chronic health needs who cannot easily reach a centre or who do not have family support.
- Bedridden elders who need ongoing home based support.
